Subclinical hypothyroidism is real, common, and missed

TSH 4-10 with normal T4 = subclinical hypothyroid. ~5-10% of adults; ~20% of women >60. Most labs flag only TSH >5.0 as 'abnormal'. But longitudinal studies show subclinical hypothyroidism with TSH 4-10 carries CV-event risk and slower metabolism. The fix isn't always meds — but it's never 'do nothing'. At minimum: optimise sleep, selenium, stress; recheck antibodies; reassess in 3-6 months.
